you can either cut a square hole in it and weld a patch into it or you can just slip a piece under the carpet .... depending on what you want to do and how far you want to go with it but as for me i would do it right and weld you a patch pannel into it

How large is a "medium size hole"? Your size and my size may differ.
I'd cut away any rust/rotted metal and weld in a new plate. Then make sure that you prep and take care of it properly. I've seen soe patch jobs last only a year or less do to people negleting prep work.

The above advice is good but what I have done in past with let's say a 2" hole is take a rotary saw and cut a hole making sure you get all the rust. Spray it with a good anti-rust compound and install a rubber floor plug.
You can do this effectively up to about 3" then you pretty much have to cut, grind, and weld.
It's a cheap fix, it's effective, and all done in about 30 minutes.
